In our professional training, we teach our carers to spot the progression of skin damage immediately. Knowing these stages can quite literally save a life:
- Stages 1 & 2: It usually starts as a persistent red mark on a "pressure point" (like the heels or lower back) where the blood supply has been restricted. If the skin breaks or looks like a shallow graze or cut, it has reached Stage 2.
- Stage 3: This is a serious escalation. At this stage, you may see yellow, "fluffy" tissue. This is dead skin that requires urgent clinical intervention to prevent infection.
- Stage 4: The most severe and dangerous stage. These sores are deep, often showing dark or black tissue, and can sometimes reach the bone. They are incredibly painful and represent a significant failure in care.

- Constant Monitoring: Checking skin integrity during every personal care visit.
